CHICAGO — Blueprint Healthcare Real Estate Advisors, a Chicago-based brokerage firm, has arranged the sale of 12 skilled nursing facilities for $134 million.

A publicly traded REIT sold the 1,324-licensed-bed portfolio. A private equity investor purchased the portfolio and entered a new triple-net lease with the existing operator.

Seven of the properties are located in New Hampshire, while five are located in Florida. Specific details on the names and locations of the facilities are not disclosed.

The seller is currently implementing a large-scale strategy that will dispose of many of its skilled nursing facilities while diversifying its operator base.

Christopher Hyldahl, Ben Firestone, Gideon Orion and Michael Segal of Blueprint handled the transaction.
